AbstractHome visits and health interventions can be successfully carried out by pharmacists. Home-based healthcare should be provided to patients who are at high risk and present high complexity. These patients are exposed to medication-related problems. Pharmacists practicing in home healthcare may partner with other healthcare team members and organizations. Pharmacists have the opportunity to apply scientific and medical knowledge to improve patients’ lives. However, the service is not widely provided as in other settings, e.g., hospital and primary healthcare. Hence, pharmacists have the opportunity to create new knowledge by researching in this area. Additionally, pharmacists have the opportunity to expand pharmacy practice into a patient’s home and ensure seamless care.
