Management of rectal lesions, benign or malignant, has evolved with the improvement of surgical tools and techniques. Total mesorectal excision (TME) and proctectomy remain the gold standard curative procedures [1, 2]. Rectal lesions are increasingly being detected with more widespread screening and awareness, and this is connected to improved tools of diagnosis.
Local excision (LE) of benign and early malignant rectal lesions was traditionally described to remove lesions within 8 cm of the anal verge [3]. Higher lesions in the rectum were rendered amenable for local excision using the transanal endoscopic microsurgery (TEM) system described by Buess et al. [4]
